Our cost to you is only $15.00 per month (US Dollars) per form that you have with us. Please understand that you are not buying Banquet Planner or any other hosted form. This is a paid subscription service with a very low monthly fee.
Everything mentioned in these articles is included. Again, no set up fees and no per change fees when you need changes made to your form, and this includes the internet hosting service.
Of course, the catch is that you have to do the design work to show us what it is you want.
